Survey Fact Sheet


Short Description
The Shedler Pain Survey (SPS) assesses pain severity and
functional impairment. The test is self-administered in
approximately 5 minutes. Physicians receive a chart-ready,
computer-generated report, printed immediately, on-site.
When to administer
Administer at initial assessment to establish baseline
pain level. Readminister periodically to monitor patient
progress. Use with all chronic pain patients.
Detailed Assessment description
The SPS meets the need for a practical, objective measure
of pain that can inform treatment decisions and provide
reliable measures of change. The instrument can be self-administered
in approximately 5 minutes, and may be used with any patient
where chronic pain is a consideration. The questions require
a sixth-grade reading level.
The Pain Intensity index measures pain severity on a standard
0 to 10 scale. Reference markers allow physicians to compare
pain level to familiar events (e.g., "the worst stomache
ache you ever had," "burning your hand on a
hot stove"). The Functional Impairment scale assesses
the degree to which pain interferes with daily activities
(e.g., occupational and social functioning). The Pain
Control scale measures perceived ability to control or
manage pain an important predictor of outcome. The
Medical Alliance scale measures the perceived quality
of the doctor-patient relationship another important
predictor of treatment outcome.
